Are you passionate about making a real difference to the
lives of people living with sight loss? Do you have experience in needs
assessment, signposting, and volunteer management? Focus Birmingham is looking
for an enthusiastic and dedicated Assessment and Signposting Coordinator
to lead our Sight Loss Support Gateway to Services Hub.
About the Role:
As the Assessment and Signposting Coordinator, you
will play a key role in developing and delivering a vital service that helps
individuals living with sight loss access the right support. You will conduct
needs assessments, provide essential information and advice, and connect people
to the right internal and external services. You will also lead and support
volunteer-led projects, ensuring our volunteer champions and other volunteers
are well-trained and supported.
Key Responsibilities:
Conduct initial customer needs
assessments using the Sight Loss MOT, ensuring individuals are supported to
live happy, healthy and fulfilling lives within their own homes and communities.
Ensure referrals are completed in
a timely manner using ‘Charity Log’.
Provide initial information and
advice over the phone.
Refer individuals to partner
agencies and community services that support their needs and contribute to
their emotional wellbeing by providing a holistic support plan.
Ensure those receiving the
service are referred to high quality information which will support them with
long term conditions.
Gather information about support
available in the community and update the services knowledge bank.
Network with organisations which
provide support to those living with and affected by visual impairments. Build
strong relationships with health, social care, and support providers.
Liaise with the marketing team to
ensure the website is updated with relevant, up to date and useful information.
Deliver a robust training
programme for our volunteer champions and other volunteers who support the
sight loss support service
Provide support and supervision
to all volunteers working in the programme, providing appropriate advice and
guidance in accordance with good practice and local policy
Undertake regular reviews with
volunteers, identify training needs and development opportunities.
Ensure that all volunteers
maintain confidentiality and act in accordance with the policies and procedures.
To lead and facilitate existing
and new volunteer led projects, provide regular updates on performance,
ensuring key performance indicators are met and services are delivered to a
high standard.
What We Are Looking For:
- Strong Knowledge of support services
provided by the third sector and other organisations in Birmingham.
- Previous
experience in assessing client needs, particularly with individuals
affected by visual impairments or long-term conditions.
- Excellent
communication and organisational skills, with a focus on providing
high-quality support.
- Good IT skills, and knowledge of any database or CRM systems.
- Commitment
to providing a responsive service in line with the departmental and
Charity-wide objectives, and in a timely manner.
- Experience
working with and supporting volunteers, providing supervision, guidance
and feedback.
- Familiarity
with relevant legislation and best practices in the sight loss sector.
- A
proactive and empathetic approach to working with people to achieve
positive outcomes and a commitment to equal opportunities and diversity.
- The
ability to identify and respond to the impact that disadvantage,
inequality, stigma and discrimination may have on individuals, communities
or groups.
- Good
understanding of safeguarding procedures and GDPR with a commitment to
promoting and safeguarding the welfare of our service users.
- Car
driver with full access to a roadworthy vehicle, with a willingness to
travel across multiple community venues.
